The NDIS and this Service AgreementThis Service Agreement is made for the purpose of providing therapy services under the participant’s NDIS plan. The parties agree that this Service Agreement is made in the context of the NDIS, which is a scheme that aims to: – Support the independence and social and economic participation of people with disability, and – Enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.
Schedule of Support The provider agrees to provide the participant therapy services: On-going group Occupational Therapy Sessions The supports and their prices are set out in the attached Schedule of Supports. All prices are GST inclusive (if applicable) and include the cost of providing the supports.
Travel For travel to a first participant appointment each day, or for travel from one participant appointment to another, therapy providers can claim up to 30 minutes of time against the appointment they are travelling to, at the hourly rate for the relevant support item. Therapy providers can also claim for return travel from the final appointment in a day.
Reports The NDIA require that therapists complete reports at the end of all services provided. Therapy Rehab charges for the time ($193.99 per hour) taken to write a therapy report that is requested by the NDIA, and claim this against the appointment at the hourly rate for the relevant support item.
Ongoing Sessions Occupational Therapist Rate In accordance to the NDIS price schedule therapy sessions Occupational Therapist hourly rate is billed at $193.99 per hour.
Occupational Therapist Assistant Rate In accordance to the NDIS price schedule therapy sessions Occupational Therapist Assistant hourly rate is billed at $86.80 per hour. Service item Therapy Assistant (Level 2 – Improved Daily Living) Working under the delegation and supervision of a therapist.
Occupational Therapist Rate In accordance to the NDIS price schedule therapy sessions Occupational Therapist hourly rate is billed at $193.99 per hour.

Cancellations –In accordance to NDIS cancellation framework Therapy Rehab Occupational Therapy Services has a short notice cancellation (or no show) we are able to recover 90% of the fee associated with the activity, subject to the terms of the service agreement with the participant.
– Cancellation is a short notice cancellation (or no show) if the participant has given less than 2 clear business days’ notice for a support that is less than 8 hours continuous duration and worth less than $1000 – There is no limit on the number of short notice cancellations (or no shows) that a provider can claim in respect of a participant.
Changes to this Service AgreementIf changes to the treatment or their delivery are required, the parties agree to discuss and review this Service Agreement. The parties agree that any changes to this Service Agreement will be in writing, signed, and dated by the parties.
